COVID-19 Vaccine Program at Lifespan Pharmacy

Lifespan Pharmacy is now offering walk-in service for the COVID vaccine, with no appointments needed, for those who are age 12 and older. There is no out-of-pocket cost to receive the vaccine.

What to Bring

Please bring to your appointment:

An ID showing your birthdate
Insurance information

On the day of your appointment, please wear loose or short sleeves to make it easier to receive your vaccine.

What to Expect When You Arrive

Patients will be screened upon arrival for COVID symptoms. Anyone experiencing COVID symptoms or who meets any other exclusion criteria will not be permitted

The pharmacy may have limited availability due to capacity and social distancing requirements.

You will be asked to remain at the pharmacy for the recommended 15 to 30 minutes post vaccine.
